Waivers & Forms

Please see below for required waivers and forms. Volunteers without all required waivers and forms will not be able to volunteer.

 

Volunteers 18 and Older

If you are 18 or older, please sign and bring the following waivers and forms with you to Sunday Morning Helpers:

·        Adult Waiver & Release of Liability

·        Adult Model Release

 

Volunteers 6-17 Years Old

Volunteers 6-17 years old must be accompanied by a parent or guardian. The parent or guardian must sign the following waivers and forms on the child’s behalf. The parent or guardian must also sign their own set of adult waivers and forms (see above). All waivers and forms must be brought by the volunteer to Sunday Morning Helpers:

·        Indemnity Waiver & Release of Liability

·        Parent Permission Slip      

·        Child Model Release

 

Parking

Underground parking is not available to volunteers. Street parking is free all day Sunday. Paid parking is available at Del Mar Station on Del Mar and Raymond.

 

Dress Code

No tank tops, shorts or flip flops. Only shirts with sleeves, pants and slip-resistant, close-toed shoes. Volunteers who are not properly dressed will not be able to volunteer.

 

What to Bring

·        All required waivers and forms

·        Water

·        Materials for crafting!! (optional but very much appreciated):

paper towel and toilet paper rolls;

cotton material for dog bandanas;

fleece;

batting/fluff;

infant socks;

dry catnip;

thick, cotton tube socks;

tennis balls;

squeakers.
